The slings are measured from the hook where the ring contacts the drum hardware, across the back, over the right shoulder, and to the point of origin. Our slings were measured on the inside (pictured on outside for clarity). There is an extra hole or two cut on the sling for a bit more adjustment in a pinch.

Shoulder measurements for the harness start from the hip, near the top of the wearer's pants. This point is roughly 7" from the center of the lower back.
The end of the measurement is the hook, where the clip attaches to the drum. In this case 37.5".
